NEED-BASED AWARD ENTRY FORM

The winning grant for the Need-Based Award will be awarded to a city or non-profit facility with demonstrated financial need. Location of splash pad must have a median household income below state median and the Parks + Recreation 5-year average capital projects budget per capita must be below $50.
